Valga municipality implements the project “Implementation of the person-centred special welfare service model in Valga municipality” in the framework of the Social Security Board's competition. The implementation of the service model is managed by the Activation Centre for the Unemployed of Valga and the duration of the project is 01.01.2022 - 31.12.2022.
In the service model, it is possible to design a personalised service for a person with a service need that takes into account his or her needs, consisting of rehabilitation, special care and social services provided by the local authority. The activities are broken down into service components that provide support in all areas of life:
- mental health maintenance and recovery,
- looking after your physical health,
- working and learning,
- leisure activities
- coping with everyday life.
In the service model, the person can reach the support offered by professionals more quickly and, as needs change, new service components can be added and support can be offered to family members. As an implementer, the local authority social worker will gain experience of the new practice of assessing the needs of people with special mental health needs and building collaborative relationships with service providers.
The project is open to people aged between 16 and retirement age with severe, profound or enduring mental health needs and is suitable for cases where the person is not eligible for state-funded special care services for some reason, or where the current service is not suitable. The service model can be implemented for 30 people in Valga municipality.
The pilot project of a person-centred special care service model based on the local government organisation will be carried out in the framework of the European Social Fund priority axis „Improving access to the labour market and preventing early exit from the labour market” sub-activity „Increasing employability of the target group of the employability reform and promoting their employment” under the activity „Piloting an integrated, person-centred and flexible special care service system”. In Valga municipality, the services are provided by Tarbatu Tervisepark OÜ, Akersoni OÜ, MTÜ Päikeseringi Selts, MTÜ Valga Abikeskus, who provide the necessary support to people and are responsible for the organisation and implementation of services.
